Medicines
A category (i) medicine is a non-prescription preparation (such as arnica cream, antiseptic liquid, insect bite treatment) that is not ingested, used for the ‘first aid’ treatment of minor injuries and provided by the service and kept in the ‘first aid’ cabinet.
Note: The service must provide specific information (including brands names) about the category (i) preparations that will be used.
To be filled in if your child requires medication as part of an individual health plan, for example for an on-going condition such as asthma or eczema etc. and is for the use of that child only.

Please indicate below whether you give permission for your child to:
ONSITE EXCURSIONS. I give my child permission to take part in on-site excursions under the conditions stated in the service’s excursions policy. Onsite excursions are times when we use spaces on the childcare centre grounds to extend learning, that are not within the licenced play space. Onsite excursions include using the carpark for riding bikes, visiting the vegetable gardens etc. The ratios for on-site excursions shall not exceed 1:4 for under two's and 1:8 for over 2’s . By agreeing to this excursions permission, you are also confirming that you have read the on-site excursions Risk Assessment and Management (RAMs) document (provided to you during the enrolment process) and agree to the RAMs plan we have in place for on-site excursions.
REGULAR EXCURSIONS. I give my child permission to take part in regular excursions under the conditions stated in the service’s excursions policy. Regular excursions will be in the local area only and are limited to: Whatawhata School, The Village Cafe complex, the Community Hall, the Golf Course and village walks using Store Road, School Road, Kura Street, Rothwell Lane and Horotiu Road. The ratios for regular excursions shall not exceed 1:4 for under two's and 1:6 for over 2’s . By agreeing to this excursions permission, you are also confirming that you have read the regular excursions Risk Assessment and Management (RAMs) document (provided to you during the enrolment process) and agree to the RAMs plan we have in place for regular excursions.
OBSERVATION FOR ASSESSMENT PURPOSES I give you permission to take photos, video recordings and written observations of my child for the purpose of documenting their learning through assessment, planning and evaluation.
BE OBSERVED BY STUDENT TEACHERS. I give permission for teachers in training to observe and photograph my child and use their photos and observations in assignments.
ONLINE PORFOLIO I give permission for Magnolia Tree Childcare to use the Storypark online portfolio platform to communicate my child's learning experiences with me. I understand this is a secure online system that is password protected and I have the ability to add other family member's access to this as well.
VISION AND HEARING NURSE. I give my child permission to be seen by the vision and hearing nurse when they are four years old and understand that it forms part of their 4 year old before school check.
SUNBLOCK. I give permission for my child to use the centre sunblock, which is a SPF50, waterproof sunblock, that has passed consumer testing for accuracy of protection claims. If you answered no, you are required to provide a named sunscreen for your child to be kept at centre.
ESSENTIAL OILS By signing this permission, you agree for the centre to use essential oils in diffusers, as per our essential oils procedure, which is provided upon enrolment. The essential oils we use are Dottera brand - Lavender, Wild Orange, Lemon and Easy air.

STATUTORY HOLIDAYS/ TERM BREAKS

This enrolment agreement is inclusive of school term breaks. 

Magnolia Tree Childcare is closed for for all the following statutory/public holidays. Full fees are payable for all statutory/public holidays, except for Christmas Day, Boxing Day, New Years Day and the Day after New Years.
